Our client's hospital offers inpatient and day programs in orthopaedics, neurological, cardiac and pulmonary rehabilitation as well as treatment programs for mood and anxiety disorders, psychosis intervention and substance abuse.

An exciting opportunity has arisen for an experience Senior Clinician to join our dynamic clinical team as one of our Discharge Planners.
This role requires effective clinical leadership, strong teamwork and organisational skills, as well as sound clinical experience.
The Location
Located in the Western Suburbs of Sydney
Essential Criteria
- A Clinical Nurse Specialist/Registered Nurse with a minimum of 5 years’ post-graduate experience (Rehab &/or Mental Health experience is welcomed)
- Current Registration with APHRA
- Previous experience in Discharge Planning desirable; knowledge of community support and services
- Ability to provide and coordinate patients with discharge services that suit their needs and requirements, while also providing education, support and guidance to patients and their families
- A strong commitment to customer service
- Initiative to drive quality projects and outcomes
- Computer literate
- Ability to work within a multidisciplinary team
- Alignment with National Standards and involvement in working parties
