The Position
The Patient Flow Access & Discharge Services Team works closely with the Clinical Coordinator and multidisciplinary teams to coordinate efficient patient journeys that are in keeping with key performance indicators and targets in relation to patient movement.
The Patient Flow Access and Discharge Services Coordinators will work in close collaboration and take direction from the Clinical Coordinator to smooth the flow of patients throughout the hospital.
You will manage:
- Patient admissions and inter-hospital transfers
- The bed allocation processes
- Admissions and inter-hospital transfers to the facility
- The priority for emergency and elective admissions access to beds in consultation with the key stakeholders
- “Right care, right time, right place, every time” including collaboration as required with the Infection Prevention and Control for allocation of a single room
- The demand and availability of inpatient beds in consultation with VMOs, Nurse Unit Managers, Midwife Unit Managers, Wardspersons, Housekeeping and others to optimise bed utilisation and flow
- Unplanned admissions / planned admissions / transfers / delays in transfers and Emergency Department status to optimise bed utilisation
- Proactive escalation to the Clinical Coordinator on any significant event
- Safe patient access and flow
- Liaison with external facilities and sub-acute services to smooth patient flow
- This includes Ambulance / Patient transport, other private hospital providers, Rehabilitation providers, Residential Aged Care Facilities (RACFs), Hostels etc
- Positive stakeholder relationships both internal and external to positively influence safe patient movement and access and promote the reputation of the hospital
- Monitoring of Elective Surgery Treatment Performance (ESAP), Emergency Treatment Performance (ETP) and NSW Ambulance Transfer of Care Performance
- Able to maintain and organise staffing profiles and rosters including the timely maintenance of Kronos
- Attendance of meetings relating to patient movement and bed allocation
The Location
Located in the Northern Suburbs of Sydney
Essential Criteria
- Current Registration with AHPRA
- Bachelor of Science (Nursing) or equivalent
- Post graduate qualification in Leadership and Management or working towards
- Previous management experience of more than 3 years
- Demonstrated effective change management skills
- Demonstrated planning and organisational skills
- Well-developed problem solving and conflict resolution skills
- Exceptional customer service skills
- Excellent interpersonal and communication skills
- High degree of computer literacy including Microsoft Office
- Prior experience in the private healthcare system will be well regarded
