The Position
As the Nurse Practitioner you will be responsible for:
- Conducting comprehensive, relevant and holistic health assessments and apply diagnostic reasoning to formulate diagnoses to inform clinical decision making and engage multidisciplinary team and patients in this process
- Advanced nursing skills in as well as extensive experience in nursing of patients with complex clinical needs conditions and social needs/issues across a variety of clinical settings
- Demonstrated effective interpersonal skills, excellent written and verbal communication skills, advanced computer skills and use of software programs to develop and deliver education
- Ensuring the provision of high quality comprehensive, individualised person centred care through a coordinated approach to care and symptom monitoring

Essential Criteria
- Current registered nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and successful completion of a Board-approved Nurse Practitioner qualification at Master’s level or educational equivalence as determined by the Board
- A post graduate qualification in Diabetes Education and Management, together with current Australian Diabetes Educators Association (ADEA) credentialing status
- Minimum 12months working experience as a Registered Nurse in Australia
- Demonstrated high level of confidence and clinical proficiency in carrying out a range of procedures, treatments and interventions that are evidence based and informed by specialist knowledge
- Demonstrated ability and confidence to apply extended practice competencies within a scope of practice that is autonomous and collaborative
- Have a strong pharmacological knowledge of diabetes medications including knowledge of insulin initiation and titration
- Effective leadership skills including highly developed skills in communication, problem solving, conflict resolution and negotiation
- Demonstrated ability to critically appraise and integrate relevant research findings in decision making about health care management and patient interventions
- Demonstrated ability in the facilitation of change management
